(Insert Form Type) - (Insert No.) Directors - (Insert SR Filing Reference) - IPN Directors listed (Insert Yes/No/NA*) - Automated reply on PPSN holders (Insert Yes/No)

For example;

• Form B1C - 3 Directors - SR1234567 - IPN Directors listed Yes - Automated reply on PPSN holders Yes
• Form B1C - 3 Directors - SR1234567 - IPN Directors listed No  - Automated reply on PPSN holders No
